Thank you for allowing our practice to provide you with your healthcare needs. Our main goal is to maintain our patients as healthy as possible. Our focus is on preventive medicine, nutritional counseling, and lifestyle modifications. We encourage patients to take an active role in their healthcare, and consider our relationship a partnership where most decisions are made jointly. We also welcome family participation when providing care for our elderly patients.

We care for adults, and adolescents 12 and older. We provide our patients with complete physical examinations, physical therapy, sports physicals, comprehensive female examinations, including endometrial biopsies and colposcopies, smoking cessation, nutritional counseling, skin cancer screening, and minor skin surgery.

 

Dr. Marie- Luise Zaldivar is Board Certified in Family Practice and a Fellow of the American Academy of Family Practice. She completed a three-year residency at Prince Georges' Hospital in Cheverly, MD. in 1995. She then served three years as a commissioned medical officer at Langley Air Force Base. She has been practicing in Johnston County since 1998 and has been in solo practice since 2001.

She serves as Medical Director for Community Home Health and Hospice, and is a certified wound care specialist. She sees patients at Johnston Wound Clinic 2 half days a week.

 

 

Our Care Coordinator is Brittney K. She maintains your electronic medical record, working with local hospitals and specialists to share and document important medical information about your health status and medical treatments. Care coordination ensures that you and all of your providers have the information needed to manage your medical conditions.

 

We partner with YOU-Turn Counseling in Clayton for mental health & substance use issues. Our co-management agreement with these behavioral health specialists ensures that we stay informed about psychological issues that may impact our patients’ physical health. For those who choose to use this service, we share information about medical conditions and recommended treatment that may impact a patients’ psychological well-being.
